It picks up sound through a microphone, processes it electronically, and delivers amplified sound to the ear canal, where it reaches the cochlea.<\/p>\n<p>For this to work, the cochlea must have functional hair cells \u2014 the sensory cells that convert sound vibrations into electrical signals for the auditory nerve. In mild to moderate hearing loss, many of these cells are present but working less efficiently. Amplification compensates by delivering a louder signal to the remaining cells.<\/p>\n<p>Hearing aids are the right choice when:<\/p>\n<ul>\n<li>The hearing loss is mild to moderate (40 to 70 dB HL)<\/li>\n<li>The child has sufficient residual hair cell function to benefit from amplification<\/li>\n<li>The hearing loss is conductive or mixed in origin<\/li>\n<li>The child&#8217;s hearing, with amplification, reaches a functional level for speech and language development<\/li>\n<\/ul>\n<p>Hearing aids are appropriately fitted when:<\/p>\n<ul>\n<li>Both ears are assessed and fitted when both have loss (binaural fitting)<\/li>\n<li>The aid is programmed specifically for the child&#8217;s audiogram \u2014 not a generic setting<\/li>\n<li>Regular audiological follow-up is done, because children&#8217;s ear canals grow and the fit changes, and the programming may need updating as the child&#8217;s hearing changes<\/li>\n<li>The family understands how to maintain the device, check battery life, and ensure consistent use<\/li>\n<\/ul>\n<p>One important caveat: a hearing aid that is prescribed but not consistently worn does almost nothing. Consistent daily use from an early age is essential for speech and language development. A child who removes the hearing aid every hour is not getting the benefit it is designed to provide.<\/p>\n<h2>How Cochlear Implants Work \u2014 And When They Are the Right Choice<\/h2>\n<p>A cochlear implant works differently. It does not amplify sound. Instead, it bypasses the damaged or absent hair cells of the cochlea entirely, converting sound into electrical signals and delivering them directly to the auditory nerve via an electrode array placed inside the cochlea.<\/p>\n<p>The device has two parts: an external speech processor (worn behind the ear) and an internal implant (placed surgically). The internal implant stays permanently; the external processor is worn daily and can be replaced or upgraded as technology improves.<\/p>\n<p>Cochlear implants are the right choice when:<\/p>\n<ul>\n<li>The hearing loss is severe to profound (70 dB HL or worse in the better ear)<\/li>\n<li>Appropriately fitted hearing aids, worn consistently for three to six months, do not provide adequate benefit for speech and language development<\/li>\n<li>The cochlea and auditory nerve are anatomically suitable for implantation (confirmed by CT and MRI)<\/li>\n<li>There is no medical contraindication to surgery<\/li>\n<li>Post-operative rehabilitation (auditory therapy and speech therapy) can be accessed<\/li>\n<\/ul>\n<p>Cochlear implants are NOT appropriate when:<\/p>\n<ul>\n<li>The hearing loss is mild to moderate and hearing aids have not been adequately trialled<\/li>\n<li>The hearing loss is primarily conductive and treatable by surgery to the middle ear<\/li>\n<li>The auditory nerve is absent or severely hypoplastic (a specific finding on MRI that makes electrical stimulation ineffective)<\/li>\n<li>The cochlea is severely malformed or ossified in a way that prevents electrode placement<\/li>\n<\/ul>\n<h2>The Trial Period: Why Hearing Aids Come First for Most Children<\/h2>\n<p>For a child diagnosed with severe to profound loss, the standard clinical pathway in India and internationally begins with hearing aid fitting, not immediate referral for cochlear implant assessment.<\/p>\n<p>The reasons for this are:<\/p>\n<p>Bilateral hearing aid fitting reveals functional potential. Some children with severe audiometric loss still have enough functional hair cells to benefit meaningfully from amplification. A proper trial with optimally fitted aids demonstrates whether this is the case.<\/p>\n<p>The trial informs cochlear implant candidacy assessment. If a child wears well-fitted hearing aids for three to six months and does not show meaningful progress in speech sound detection, vocalisation, and early word development, this is evidence of inadequate hearing aid benefit \u2014 which is a core criterion for cochlear implant candidacy.<\/p>\n<p>Age matters. The shorter the trial, the earlier the implant can be placed within the critical developmental window. This is why the assessment should begin immediately after diagnosis, not after a year of observation. Cochlear implant \u2014 comparable to hearing peers in most children implanted before age 2.<\/p>\n<p><strong>Reversibility:<\/strong> Hearing aid \u2014 fully reversible. Cochlear implant \u2014 internal implant is permanent; external processor can be removed.<\/p>\n<p><strong>Upgradeability:<\/strong> Hearing aid \u2014 new aids can be fitted at any time. Cochlear implant \u2014 external processor can be upgraded; internal implant stays.<\/p>\n<p><strong>Minimum age:<\/strong> Hearing aid \u2014 any age including newborns. Cochlear implant \u2014 typically 12 months, some centres 9 months.<\/p>\n<h2>The Role of Auditory Neuropathy Spectrum Disorder<\/h2>\n<p>There is a specific condition that does not fit neatly into the hearing aid vs cochlear implant comparison: auditory neuropathy spectrum disorder (ANSD).<\/p>\n<p>In ANSD, the cochlear hair cells function (OAE testing is normal or near-normal), but the signal transmission along the auditory nerve is disrupted.
